CultNetWriteCompletion.SendAndAwaitResultAsync registers a refusal handler before
the send, then waits a result window (default 500 ms; CultMeshClientOptions.
SubmitResultWindow, CultNetSchemaWriteForwarderOptions.ResultWindow). A
cultnet.error.v0 inside it rejects with the new CultNetWriteRefusedException
(Code: write_not_authorized, message_invalid, ...) whatever its order against the
send or transport ack; a send returning no longer completes the write. The
forwarder's refusal now throws on the same path as an unreachable primary, so
CultNetDatabaseServer.TryForwardPutAsync/DeleteAsync no longer report a refused
forward as done. Raw send primitives (C# schema client/session, Python, Kotlin)
carry the doc line: fire-and-forget, an ack or a return is not admission, read the
result frame.

One loop now serves both: every put acknowledged, then a refusal frame (typed
cultnet.error.v0) fails the call in either order against the ack, and a success
frame or the end of the new result_window option (default 500 ms) completes it.
The batch no longer waits on flush_reliable alone, so it cannot hide a refusal.
Raw send primitives (CultNetRudpSocketTransportConnection::send/send_reliable/
flush_reliable, tcp_framed send) carry the doc line: fire-and-forget, an ack or
a return is not admission, read the result frame.

The stub's 120 ms second packet fired after peer.close() and threw
ERR_SOCKET_DGRAM_NOT_RUNNING as an uncaught exception, failing whichever
publisher test was running when the timer landed (load-dependent). Timers are
now cleared in the test's after hook instead of swallowing the error.

7082c9488b cultmesh-ts: a put completes on its result frame, not on the transport ack
publishRudpDocumentOnce resolved when peer.flush returned (ack only) and read no frame, so a
refused put resolved. It now rejects with CultMeshRudpWriteRefusedError (the peer's typed
cultnet.error.v0: write_not_authorized, message_invalid) whether the refusal arrives before the ack,
after it, or within resultWindowMs (default 500 ms) after it; a success reply ends the wait early.
publishRudpDocumentAndWaitForReceipt rejects with the same typed error instead of timing out.

A Ping in the 2 s grace after the capped frame was answered with a Pong built from the
session's ack state, which includes the capped frame. Every reply while the session is
ending now zeroes its ack fields. The deadline no longer wraps at the end of the clock.

The schema server answered the 8th undecodable frame and ended the peer's session in the same
call; ending clears the session's pending reliable packets, so a lost datagram was never
resent. The session now stays until the peer has acknowledged every answer (a lost one is
resent by PollResends) or a 2 s grace runs out, serves nothing after the capped frame
meanwhile, and then sends the goodbye.

The capped (8th) refusal went out unreliable and unordered with the goodbye at once, so a
lost datagram left the peer without it. It is now queued like any response and resent until
the peer acknowledges it; the session ends then (or after a 2 s grace) and serves nothing
after the capped frame meanwhile. The answer, its resends and the poll that carried the
capped frame acknowledge nothing, as every refusal. A reliable queue that cannot take the
answer ends the session at once with the unreliable refusal, as before.

A map with an unknown schemaVersion was rethrown: unanswered, uncounted toward the

unreadable frame, so the cap holds for it.

Heimdall's parity ruling. The schema server ended a peer's session on the first
frame it could not decode. It now answers message_invalid (field "message") and
keeps the session; RudpCultNetSchemaServer.MaxConsecutiveRefusedFrames (8)
consecutive undecodable frames end it after the last is answered, and any
decoded frame resets the run. Handler faults still end the session at once.

An undecodable schema frame was dropped and a DocumentDelete ignored, both
silently. Now the first is refused message_invalid (details.field "message")
and the second write_not_authorized naming the record, each with a fixed
reason, ending the peer's session as the other refusals do. The sink gate is
unchanged. Both bytes are pinned against the shared error-vectors.json.

"""Heimdall's invariant: no client treats a reliable-transport ack as write admission. A put completes only on
its result frame (write_not_authorized, or the success reply).
The Python write path is a subscription over RUDP: `send` queues the put, `read_next` returns the result frame
and raises the peer's refusal. The server here refuses the put; its reliable ack of the put arrives (the test
waits for it) and the client still reports the refusal, never success. The transport's `receive` yields data
frames only, so the order of the ack against the refusal cannot change what `read_next` reports.

/// Unlike [`publish_cultnet_message_to_rudp_catalog`], which polls for
/// per-message application-level acknowledgement, this sends every message on
/// one connection and waits only for a transport-level `flush_reliable`. It
/// trades per-receipt confirmation for batch throughput; callers that need a
/// stronger delivery guarantee should use the singular publish path instead.
/// Completes under the same rule as [`publish_cultnet_message_to_rudp_catalog`]:
/// every message acknowledged, then no refusal frame within `result_window`
/// (or one success frame per message). A refusal of any message in the batch
/// fails the whole call; the batch cannot hide it. The catalog's answers do not
/// name the message they answer, so a failed batch does not say which message
/// was refused or whether the others were admitted.

### Added
- `createRudpDocumentServer` ends a session after `CULTMESH_RUDP_DOCUMENT_SERVER_MAX_CONSECUTIVE_REFUSED_FRAMES` (8) consecutive undecodable frames or consecutive deletes the write authority refuses; fewer are answered and the session is kept. Any decoded frame resets the undecodable run, any decoded non-refused-delete frame resets the delete run. The same cap holds in every runtime.
- `CultMeshCatalogValue<TDocument, TSchema>`, exported.
- `documentById` and `tryDocumentById` on `CultMeshDocumentCatalog`, which address one document by its document id when a catalog holds several documents of one type.
